It depends whether you’ll be applying to a new school for the Ph.D. or just rolling your Master’s studies right into the Ph.D. program at your current school. Just follow the application instructions. The most intimidating part is the “quals”, usually around your second year. A verbal exam to see if you can continue in the program.
In most programs you take classes for a couple of years, pass comprehensive exams (form depends on the school – sometimes verbal, sometimes written, often both) and then you get to write your dissertation. My masters comps weren’t tough (I was really well prepared), but I’ve heard many a horror story about the PhD comps. Rules on how you defend your dissertation also vary. Some of my profs didn’t have to defend because they’d already taken jobs far from their campus, but that wasn’t true for everybody.
